Patient Rights
Patients have these rights without regard to cultural, economic, educational or religious background, gender, or source of payment for care.
You have the right to:
- Considerate and respectful care.
- Physician’s name and professional relationships of other physicians responsible for coordinating your care.
- Receive information about your illness, treatment and recovery in terms you can understand.
- Receive as much information about your procedure or treatment in order to give consent or refuse treatment, except in emergencies.
- Participate in decisions regarding your procedure or treatment. This includes the right to refuse treatment, to the extent permitted by law.
- Privacy concerning your treatment, procedure, consultation, examination and medical records.
- Know the reason for the presence of any individual involved in your care.
- Copies of your medical records. Communications and records pertaining to your care will remain in this facility. Written permission must be obtained from you before medical records can be made available to anyone not directly concerned with your care.
- Receive a reasonable response to any reasonable request for service.
- Leave Fairway Medical, even against the advice of your physician, after signing the Against Medical Advice form.
